MeridianRx collects and uses certain personal information from you when you use our website, online portal, live chat service, mobile application and other online services (collectively, “Online Services”). If you use the Live Chat service, we use the information you type in to help answer your question. We use this information for training purposes. Training helps MeridianRx give you better service.
Our online portal (“Portal”) can provide you with important and useful health information, including health support tools and other services. Because this information is catered to you, we require you to create an account to access the Portal to help us protect your information and ensure only you have access to appropriate information on the Portal.
The first time you use the Portal we will ask for your name, email address, ID number, health plan or plan sponsor, and date of birth. We use this information to verify your identity. We also use it to give you a user name and password for your online account.
Our mobile application (“App”) can provide you with important and useful health information, including health support tools and other services, much like the Portal. Because this information is catered to you, we require you to create an account to access the App to help us protect your information and ensure that the information available to you aligns with your drug plan.
The first time you use the App we will ask for your name, email address, ID number, health plan or plan sponsor, and date of birth. We use this information to verify your identity. We also use it to give you a user name and password for your online account.
In addition to these, the App captures your contact information, including your name, contact image, email address and phone number from your phone in order to personalize your experience. Additionally, the App can use your location information to show you pharmacies and provide other geolocation services. You can consent to these permissions when you install the App from your phone’s application store. The App does not share this information with MeridianRx and the information remains on your phone while the App is running and then is cleared when you uninstall the App.

MeridianRx believes that coordination and cooperation are essential to achieving positive health outcomes. MeridianRx provides links to information that we believe you may find useful in reaching your health goals. MeridianRx does not own or control these sites and cannot guarantee your privacy or security when visiting these third party sites. We do not endorse any third party website, even if a link is provided on our website. MeridianRx cannot guarantee the quality or accuracy of information provided by third parties.
The MeridianRx Online Services are designed to be used by individuals who are at least 18 years old. Any information provided to us for a person under 13 years of age must be provided by a parent or guardian. If you are a parent or guardian of a minor under 13 years of age and you believe that the minor’s information was provided to MeridianRx through these Online Services, you may request that we delete that information from our systems. Please contact us at firstname.lastname@example.org and be prepared to verify your identity as the minor’s parent or guardian as part of the request.
Be safe online and watch out for fraudulent websites, spam, and phishing, including fake job postings. Fraudulent websites may use the MeridianRx and its affiliate organizations’ logos and tradenames illegally and often for the purpose of defrauding individuals. MeridianRx and its affiliates take efforts to have these sites and spam shut down.
You can help protect yourself by being aware of the following: MeridianRx will only contact you from email addresses ending in @mhplan.com or @meridianrx.com. MeridianRx does not conduct interviews using Google Hangout, G-Chat, Face Time, Facebook Messages or any other third-party chat room. MeridianRx will never ask for your financial or private information prior to extending a job offer. Throughout our website, there may be opportunities to contact MeridianRx via electronic mail or "email." Most email is unencrypted. This means that it can be more easily seen by other internet users while it is being sent, without the sender's permission. The sender may not know if someone else sees it. MeridianRx can encrypt the email that we send you that helps to limit this risk.
To protect your privacy, please avoid using email to send MeridianRx private or confidential information unless that email is encrypted. MeridianRx cannot guarantee the confidentiality of any email you send to us through the internet. Use of email is solely at your own risk.
Notice of Privacy Practices
MeridianRx is often considered a Business Associate of its various clients under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy and Security Rules. In accordance with these regulations, MeridianRx is required to operate in accordance with its clients’ Notice of Privacy Practices which describes member rights and our obligations with regards to individually identifiable health information (often called PHI).
